Initially, an individual or caregiver will make contact with our team and we will obtain relevant background information on the client as part of the referral and on-boarding process. We start with a face-to-face assessment, which typically lasts between 1 to 2 hours. Based on the needs and goals of the individual, the assessment may draw on a variety of standardised assessment tools along with clinical observations to identify a clear picture of the client’s strengths and areas requiring support.
Depending on the needs of the individual or the care team, we may write a detailed report or prepare an intervention action plan to map out the client’s goals and required interventions. This will guide the development of targeted, evidence-based intervention strategies personalised to the individual.
